Medical Mall Model: a consideration for Ghana?

I bring you another interesting chapter in my new out doored book on Black Men & Prostate Health! There are interesting amazing functions of the prostate gland I know you have not heard or read anywhere. To start with, in the course of my research into black men and prostate health, I realized that, the more scientific name for an enlarged prostate is BPH, or benign prostatic hyperplasia.

“Benign” refers to the fact that the condition is not cancerous or malignant. “Prostatic” refers to the prostate gland. “Hyperplasia” is the growth of tissue through an increase in the number of cells. Men develop an enlarged prostate either because they experience an increase in the number of cells growing in the prostate, or there is a decrease in the number of cells that die.

Technically, BPH also is associated with two other prostate conditions, prostatitis and prostate cancer, since both diseases involve abnormal growth of the prostate. Sometimes you may see the term benign prostatic hypertrophy, which some doctors and laypeople use when referring to the same condition, but technically it is a misnomer. “Hypertrophy” means the growth of tissue through enlargement of existing cells.

This is why you need to read my close to 900-page book on the prostate now for well informed education on healthy prostate.

God is God! And he knew exactly why he created this small gland, though, not vital for life!  But seeing so many black men rushing these days just to save their prostate, means, it is an important thing for black men!  I consider the prostate to be the most useful and powerful aspect of the man’s reproductive life!

This is why I termed it the ‘Powerhouse’ or the ‘central nervous system’ of every man! Though, women, do not have prostate gland, they faced several challenges if the man’s prostate is in danger! Why? Because, the man’s sex life could be in danger and ultimately affect the woman sex life as well. The man would deprive or starve the woman her priciest need in life-SEX! No woman would want to see her husband not being able to satisfied her sexually or unable to make babies with! There are so many functions of the prostate gland.

To start with, it secretes a fluid that keeps sperm alive while protecting them and the genetic code they carry. The prostate contracts during ejaculation and squirts its fluid into the urethra. During ejaculation, sperm travels along two tubes called vas deferens; they carry millions of sperm from the testes (where they are made) to the seminal vesicles. The seminal vesicles are attached to the prostate and add extra fluid to the semen before it is sent down the urethra.

The site where the vas deferens meets the seminal vesicles is known as the ejaculatory duct. The prostate is also a valve and functions by contracting during ejaculation, closing off the opening between the bladder and urethra and pushing semen through at speed. This is why it is impossible to urinate and ejaculate at the same time.

You see, as it surrounds the upper part of the urethra tube just below the bladder (the prostatic urethra), its controls the flow of urine. It does this by preventing urine from exit the bladder, apart from when unrestricted by urination. It also prevents urine from destructive ejaculate during orgasm. It does this in a very critical way by using the two small prostatic muscles called sphincters.

They act as soldiers or gatekeepers with guns or shut-off valves to control and regulate the dual-purpose urethra tube. These soldiers or gatekeepers make sure the right fluids flow at the right time – urination or ejaculation. One sphincter is situated where the bladder and the upper part of the prostate convene (the interior upper sphincter).

In a healthy state, it prevents urination until the man feels the need to go and pee and stops seminal fluid from moving backwards into the bladder during ejaculation. When injured, semen is pushed back into the bladder and finally exits with normal urination. This is known as retrograde ejaculation because the normal is called ante grade ejaculation and is an added possible side effect of prostate surgery – no chance of seeding a woman then according to Ronald M Bazar.

The second, external lower sphincter is at the bottom of the prostate and is subject to our control. It prevents dribbling after urinating and is how as men voluntarily can delay urination when ill-timed to go. Incontinence finally occurs when control of either sphincter is damaged and urine leaks or flows uncontrollably, this force many men with prostate problems to wear adult diapers.

But men can voluntarily control the lower sphincter and to stop urine or semen from moving if men have enough Kegel muscle control, the ability to squeeze the flow shut. Any one of these sphincter muscles will block the urine until the push for to peeing takes control and the timing is right to release and let the urine flow.

An enlarged prostate or BPH can squeeze the prostatic urethra tube and the upper or lower sphincter, making urination difficult with a host of unpleasant, uncontrollable symptoms. BPH surgeries that remove part of the prostate can easily have side effects of incontinence or retrograde ejaculation.

It also produces Prostatic fluid, the milky fluid produced by the prostate during ejaculation makes up around 30 percent of the total fluid ejaculated (the rest is sperm and fluid from the seminal vesicles). Prostatic fluid protects sperm, helping them live longer and be more mobile. It contains a number of ingredients, including enzymes, zinc, fructose and citric acid that actually give sperm energy to make the journey to the egg. Semen also contains substances like antibodies that may protect the urinary tract and sperm from bacteria and other pathogens.

This is why too much frequent sex depletes the man of zinc and could be detrimental to the man’s health. You need the right education on sex and prostate health and not fall for the 21 times craze being currently promoted.

In my book, I wrote extensively on sex and prostate health based on science and my recommendations. One of the enzymes in prostatic fluid is prostate-specific antigen (PSA); after ejaculation, PSA makes thickened semen softer or liquefied the glued semen, helping sperm travel through it more easily, increasing their likelihood of successfully fertilizing an egg.

Healthy semen is the perfect consistency and environment for sperm transit, survival, and for fertilization thus our semen includes enzymes like PSA (which is often measured as part of screening for prostate cancer), as well as other substances made by the seminal vesicles and prostate.

Although prostatic fluid is slightly acidic, the other components of semen make it alkaline overall; this is to counteract the acidity of the vagina and protect the sperm from damage. It is therefore detrimental if a man’s prostatic fluid is highly acidic in nature, it could lead to infertility in male. This is why during Semen analysis; the pH of a man’s semen is analyzed to check for acidity or alkalinity.

The prostate naturally grows during puberty, not by itself but under the control of the male hormone testosterone and its byproduct dihydrotestosterone (DHT). Testosterone is primarily made in the testes, but a smaller amount is also made in the adrenal glands above your kidneys. This is why if the prostate is removed during surgery, you could still secrete some amount of Testosterone.

However, the conversion of testosterone to dihydrotestosterone is not done by the testosterone itself, so by what means? The prostate also contains a vital enzyme, 5-alphareductase. This enzyme converts the hormone testosterone in the body to DHT (dihydrotestosterone), which is at least ten to thirty times more powerful than simple testosterone.

This potent hormone DHT has numerous function including male sexual drive and function. As men age and at a point in time, a build-up of toxins in the prostate may affect the production of this enzyme, which is then accountable for the declining sex drive in men as they age.

DHT and testosterone have erroneously been under attack by the Medical Community as responsible hormones in prostate problems rather than the extreme rise in modern male estrogen levels, leading to medical interventions with serious side effects including lack of libido.

Estrogen levels rise because of the predominance of estrogens in factory foods, commercial meats and dairy and estrogen-mimicking chemicals present in bodycare and household products. It’s even found in our water and some plastic food packaging. The growing trend of more Gays in black men could also be linked to the prostate location; inside the rectum which is the G-Spot. Since doctors insert their fingers into the prostate to check for any prostate abnormality with a gel, penetration inside the rectum also could touch the prostate and produces an extreme sexual excitement.

The Prostate is the G-Spot, as stimulation can create an extraordinarily well-built sexual response and strong orgasm in men that are open to this sexual method. So your ability to manage ejaculation at the prostate can also guide you to delayed orgasms and “ejaculations” where no semen is moved out-so men suffering from premature ejaculation you happy now?

This could also be accounted to the fact that more black men do not prefer prostate digital rectal examination from female docs as it could lead them on. But the most important work of the prostate which is really vital to me is its ability to sieve and do away with toxins. You can apply this by the way our women use sieves in the kitchen. That is exactly the work of the prostate too.

By acting as a sieve and removes toxins or waste to defend the sperm, which helps men’s chances of impregnation and also made sure that men start with the most favorable class of sperm. I consider this as the prostate’s most significant function and, at the same time, can be one of the main reasons there is a rising problem of prostate disease and cancer as men in Ghana and the black communities deal with more and more toxins or wastes in our food, water and the surroundings.

The prostate gland is a very complex gland with numerous functions; if you don’t take good care of it then certain prostate disease can cause havoc on your health. So I entreat you as men, to do everything possible now that you know the functions of the prostate gland to be smart and pay close attention to your prostate gland likewise our women as they play a critical role in ensuring the health of our prostate gland.

You see, a detrimental prostate can have a huge impact on your sexual function and your daily urination. So you can describe it any way you want it but if you don’t take good care of it then you are in doom and your ability to enjoy good sex has now been term ‘sleeping on duty’! This is just what to expect in my latest close to 900-page book on the prostate. The book is out for GH¢500 only. Enquiries call (0241083423/0541234556). It is a well research book on the prostate health. Thank you.
